Dei recettori Beta-adrenergici agenti di blocco (aka beta-bloccanti), in generale, non deve essere usato in pazienti con conclamata insufficienza cardiaca congestizia (CHF). Stimolazione simpatica potrebbe essere importante per il mantenimento emodinamica in funzione in questi pazienti, in modo da beta-blocco può peggiorare l'insufficienza cardiaca. Tuttavia, la terapia con beta-bloccanti può essere utile e può essere somministrato con cautela in alcuni pazienti CHF condizione che siano ben compensata e la ricezione digitale, diuretici, ACE inibitore e/o nitrati. Il carvedilolo, in particolare, è indicato per l'uso con questi agenti nel trattamento di entità da lieve a grave insufficienza cardiaca di origine ischemica o cardiomiopatica. C'è anche una crescente evidenza che l'aggiunta di un beta-bloccante alla terapia standard, possono migliorare la morbilità e la mortalità nei pazienti con insufficienza cardiaca avanzata, anche se è incerto se l'efficacia varia in modo significativo con i diversi agenti.
