Consumatore:

Alcune condizioni sono state identificate come cause della riduzione della clearance della teofillina. Essi includono l'età (neonati e bambini < 1 anno, così come i pazienti anziani > 60 anni) e le seguenti patologie concomitanti: edema polmonare acuto; insufficienza cardiaca scompensata; cor polmonare, febbre (>= 102 gradi per 24 ore o più, o meno elevazioni di temperatura per lunghi periodi); influenza; non trattata o non controllata ipotiroidismo; malattie del fegato, cirrosi epatica o epatiti acute; ridotta funzione renale in bambini < 3 mesi di età; sepsi con insufficienza multi-organo; e shock. La terapia con teofillina deve essere somministrato con cautela nei pazienti che presentano uno o più di questi fattori di rischio, e il dosaggio deve essere opportunamente ridotto per prevenire la tossicità. Un monitoraggio più intensivo di siero concentrazioni di teofillina può essere richiesto. La tossicità è più probabile che si verificano quando i livelli di superare i 20 mcg/mL. Casi più gravi, a volte, senza preavviso, hanno portato ad aritmie cardiache, epilessia intrattabile, e la morte.
