Consumatore:

Dei recettori Beta-adrenergici agenti di blocco (aka beta-bloccanti), in generale, non deve essere usato in pazienti con conclamata insufficienza cardiaca congestizia (CHF). Stimolazione simpatica potrebbe essere importante per il mantenimento emodinamica in funzione in questi pazienti, in modo da beta-blocco può peggiorare l'insufficienza cardiaca. Tuttavia, la terapia con beta-bloccanti può essere utile e può essere somministrato con cautela in alcuni pazienti CHF condizione che siano ben compensata e la ricezione digitale, diuretici, ACE inibitore e/o nitrati. Il carvedilolo, in particolare, è indicato per l'uso con questi agenti nel trattamento di entità da lieve a grave insufficienza cardiaca di origine ischemica o cardiomiopatica. C'è anche una crescente evidenza che l'aggiunta di un beta-bloccante alla terapia standard, possono migliorare la morbilità e la mortalità nei pazienti con insufficienza cardiaca avanzata, anche se è incerto se l'efficacia varia in modo significativo con i diversi agenti. I dati da una meta-analisi di studi suggeriscono una maggiore riduzione del rischio di mortalità per non selettivi beta-bloccanti che per la beta-1 selettivo agenti.

Del recettore dell'angiotensina II (AR) antagonisti possono causare insufficienza renale in pazienti la cui funzione renale dipende dall'attività del sistema renina-angiotensina-aldosterone. Inoltre, ipotensione sintomatica può verificarsi in individui sensibili, che potrebbero compromettere la funzionalità renale ed perfusione del miocardio. In pazienti con grave insufficienza cardiaca congestizia (CHF), il trattamento con AR antagonisti è stato associato con oliguria e/o progressiva azotemia e, raramente, insufficienza renale, l'ischemia del miocardio e morte. La terapia con AR antagonisti deve essere iniziato con cautela in pazienti con grave CHF, specialmente se accompagnati da un volume e/o deplezione di sodio. In pazienti che soffrono di una diminuzione della funzione renale, l'interruzione di AR antagonista terapia è di solito non sono necessarie nel caso in cui non vi è miglioramento sintomatico dell'insufficienza cardiaca e renale, peggioramento è ben tollerato. Ipotensione transitoria non è una controindicazione per un ulteriore trattamento con AR antagonisti, dal momento che la terapia di solito può essere ripristinato senza difficoltà dopo la pressione del sangue si stabilizza.
